You’ll need the Chick-fil-A survey code printed at the bottom. If there’s no code, it means that visit isn’t eligible.
Visit the official survey page
Go to MyCFAVisit.com, choose your language, and enter the survey code to begin.
Answer a few questions about your visit
You’ll rate things like:
- Speed and friendliness of service
- Accuracy of your order
- Cleanliness of the restaurant
- How likely you are to return
Write down your validation code
When you finish, you’ll receive a validation code to write on your receipt. Bring it back within 30 days to redeem your free Chick-fil-A sandwich.

I did the survey on July 24, 2023 and have yet to receive my free coupon code. I am very disappointed after I spent the time filling out the survey.

I visited Luther store in Metro Center in Nashville, Tennessee. Order #5460719. I ordered $39.25 worth of food but did not get one of my salads. I tried calling the store when it was discovered that my order was incomplete. I would like my money back for only getting one of the salads.

What required serial number? Where is it? It is not on the receipt. It is not my pleasure for eating at a establishment that does value their customers. Zaxbys have serial numbers on their receipts and value their customers.

I got fed up trying to do the survey, spent all that time filling out the survey just for them to tell me over and over again that I wasn’t putting in my correct email very much dissatisfied with the survey let see after this survey if I have the correct email

Placed a order yesterday and tried many times today to enter the required serial number. Each time unable to do so because of ‘lack of information!’!! Very disappointed by what should have been a simple task became impossible. I LOVE Chic-fil-A and have since having my first one many years ago! Still love it no matter what.
We had the same issue with the serial number not working on the survey the next day. Not really concerned with the free sandwich, but was very disappointed that the order was not accurate and did not get all my items when I arrived at my house. Did not receive the sauces or the added condiments on one sandwich.

Last night I completed my receipt survey,but by mistake I entered the wrong mail,how do I recuperate my code?
I placed an order on 11/21/2019 for our customer. Amount of the charge was $185.21, not a small amount for us to spend. On the receipt was an offer for a free Chicken Sandwich as a thank you. I tried numerous times to enter the serial number and it never worked. I am truly disappointed that your company fell through on the offer!
